Abstract  The paper acknowledges the benefits of casemix-based funding while enumerating the main concerns of clinicians. Casemix is seen by clinicians as being a more equitable way of funding than historical allocation and a valuable source of data for service provision planning. The main concerns are the inadequacy of the diagnosis related group (DRG) classification system and the absence of adequate baseline data to allow accurate assessments of the impact of casemix-based funding on quality of care; the lack of direct recognition within DRGs of the training, research and development roles of public hospitals; and the instability of an evolving funding and classification system which could impact negatively on the sta bility of hospital budgets and thus service provision for Victorians in the future.
